Insurances Currently Accepted for Ohio based clients:
Anthem/BCBS until October 20, 2026
United HealthCare/UMR/Surest and Caresource until December 31, 2026
Self Pay: $145/intake, $135/subsequent
As I am not in network with insurance companies, I will provide a Good Faith Estimate following the intake, and will provide a receipt and super bill after each appointment in order for you to submit to your insurance company for out of network (OON) benefits.
Accepted forms of payment: HSA/FSA, credit, debit, check, cash

*Rates may be negotiated on the basis of financial need (such as having a Medicaid or Medicare based insurance, job insecurity, etc) as well as for certain professions; active or retired military personal and their families, first responders, health care employees, and educators.
